This privacy policy (the “Privacy Policy”) discloses the information gathering and dissemination practices for CertaPet LLC, the owner of this website (CertaPet.com). CertaPet LLC is required to abide by the terms of the current Privacy Policy. We reserve the right to modify our privacy practices, which may apply to existing and subsequently collected health information. A copy of the current notice is posted at CertaPet.com. We will notify you of changes by posting the new policy on this website. Applicable law requires CertaPet LLC to maintain the privacy of your protected health information and to provide you with notice of our legal duties and privacy practices with respect to protected health information.
Your authorization of these practices is essential for us to provide our services. We need you to provide accurate information so that we can conduct a preliminary screening to determine whether to connect you with one of the doctors or psychiatrists that we work with, who will assess whether to make a written recommendation for an emotional support animal. You can opt-out and decide not to provide the requested personal information. However, by doing so, we will be unable to conduct our screening and you will not be contacted regarding a written recommendation. You can also request that we not use any information you submitted by clicking emailing us at privacy(at)certapet.com.
We use the information we collect to determine whether to connect you with a doctor or psychiatrist, who will assess whether to make a written recommendation for an emotional support animal. We obtain your written authorization to permit us to use your personal health information this way.
We do not sell your name or personal information, but may use your name and email address to send you advertisements.
When you pay for anything on our site using a credit or debit card, we collect the personal information needed to complete the transaction. This information includes your name, account number, and other information necessary to process your payment, all of which is clearly labeled on the form you use to submit payment. We use an unaffiliated, payment processing company to process your payment. We transfer your encrypted payment information—and you authorize us to do so—to this company solely for this purpose.
We automatically collect certain information from visitors to the site, such as Internet addresses, browser type, Internet Service Provider (ISP), referring and exit page, operating system, timestamps, and clickstream data. We also track and analyze non-identifying and aggregate usage and volume statistical information from our visitors and customers. This information is logged to help diagnose technical problems, and to administer our site so that we can constantly improve the quality of the services we provide. We take measures to prevent the loss, misuse, and alteration of your information. We carefully limit access to the database in which your personal information is stored in compliance with HIPAA. Additionally, we share and transfer your personal information only in the ways set forth in this privacy policy.
We can (and you authorize us to) disclose any information about you to law enforcement or other government officials as we, in our sole discretion, believe necessary or appropriate in connection with an investigation of fraud, intellectual property infringements, or other activity that is illegal or may expose us to legal liability.
This website is directed at teenagers and adults and not to children under the age of 13. We do not knowingly collect personally identifiable information from children under the age of 13, nor do we knowingly distribute such information. We do use the services Hotjar and Lytics in order to better understand our users’ needs and to optimize the service and experience we provide.
Hotjar and Lytics are technology services that helps us better understand our users’ experience (e.g. how much time they spend on which pages, which links they choose to click, what users do and don’t like, etc.) and this enables us to build and maintain our service with user feedback. Our service providers store this information on our behalf in a pseudonymized user profile and they are contractually forbidden to sell any of the data collected on our behalf.   • Health related information that you share with us during the onboarding process and during your meeting with mental health professionals, is maintained under strict compliance with HIPAA.
  • Contact information, such as your postal address, email address and telephone number;
  • Profile information, such as your username and password, interests, preferences, feedback and survey responses;
  • Feedback and correspondence, such as the information you provide in the survey responses, when you participate in market research, report a problem, receive customer support or otherwise correspond with us;
  • Transaction information, such as your credit card or other payment and billing details;
  • Usage information, such as information about how you use the Service and interact with us; and
  • Marketing information, such as your preferences for receiving marketing communications and details about how you engage with them.

+ If you are writing to us from the email address associated with your CertaPet account or transactions that CertaPet has on file for you, then to complete the verification process, we will ask you to confirm the action you wish us to take before we proceed (such as confirming you would like us to delete your information).
+ If you are not writing to us from the email address associated with your CertaPet account or the transactions that CertaPet has on file for you, then to complete the verification process, we will ask you to provide 3 specific pieces of data that we have on file for you, such as email address you used when making a purchase or billing information such as the last 4 digits of the credit card used or the billing address for the credit card.
***Note the “Right to Delete” does not apply to information required to be retained for compliance with HIPAA or other mental health regulations.

If you are exercising your Right to Know or a similar right that does require disclosure of specific pieces of information about you…
+ If you are writing to us from the email address associated with your CertaPet account or transactions that CertaPet  has on file for you, then to complete the verification process, we will ask you to both: 

  1. Confirm the action you wish us to take before we proceed, and 
  2. Provide a statement that you “declare under penalty of perjury that you are the person about whom the Right to Know request has been made.”

+ If you are not writing to us from the email address associated with your CertaPet account or the transactions that CertaPet has on file for you, then to complete the verification process, we will ask you to both: 

  1. Provide 3 specific pieces of data that we have on file for you, such as email address you used when making a transaction or billing information such as the last 4 digits of the credit card used or the billing address for a transaction, and 
  2. Provide a statement that you “declare under penalty of perjury that you are the person about whom the Right to Know request has been made.”

You are also permitted to designate an authorized agent to submit certain requests on your behalf. In order for an authorized agent to be verified, you must provide the authorized agent with signed, written permission to make such requests or a power of attorney. We may also follow up with you to verify your identity according to the above methods before processing the authorized agent’s request.
If we are not able to successfully verify you according to the above methods, then we will not be able to comply with your request.
CertaPet will not discriminate against you if you exercise your rights under CCPA. However, if your request requires us to delete, restrict or not share information, please be advised that this will inhibit some functionality of the Site. Any subsequent interaction with the Site after deletion will require new requests for action on your data.
